Strengthening Milk Donation Programs: A Collaborative Success Story

Title: Impact of involvement of NGO in strengthening breast feeding practices and milk donation in a resource limited health care facility


🌼 Why Breastfeeding Matters

Early initiation and exclusive breastfeeding during the first six months are vital for newborn survival, growth, and immunity. It remains one of the most cost-effective strategies to reduce neonatal mortality rate (NMR) and infant mortality rate (IMR). Yet, in resource-limited hospitals, short post-delivery stays and limited staff make lactation establishment difficult.


🏥 Setting the Scene

This initiative was conducted at Chhatrapati Shivaji Maharaj Hospital (CSMH), a 500-bedded tertiary care hospital in Kalwa, Thane. The hospital operates its own human milk bank, collecting and distributing donor milk to neonates in need.

However, early discharge policies (48 hours for vaginal delivery and 5 days for caesarean section) limited mothers’ time to establish breastfeeding confidence.


🤝 The Intervention

To bridge this gap, a mother-support group was formed. Trained counsellors—particularly experienced elderly women—provided emotional support, practical lactation guidance, and counselling on milk donation. This peer-support approach helped dispel misconceptions and reduce unnecessary formula feeding.


📊 What Changed?

Over 299 days:

  • 👩🍼 3,490 mothers counselled
  • 🫶 1,389 donors registered
  • 🍼 265 newborns received donor milk
  • 🥛 123.54 litres of milk collected

Between 2016 and 2018:

  • Milk collection rose from 31.06 L to 83.25 L annually
  • Donor registrations increased from 472 to 1,128
  • Newborn recipients increased from 52 to 179
  • 📉 Significant reduction in illness-related hospital visits in the first six months of life

🌍 Key Takeaway

In resource-limited settings, community-based lactation support groups can significantly strengthen breastfeeding practices and milk donation systems. Empowering mothers through structured counselling and peer engagement is a sustainable model to improve neonatal health outcomes and reduce infection-related morbidity.

Dr. Sandhya Khadse is  a Paediatrician specializing in Neonatal Nutrition and Human Milk Banking, she holds a MD degree and she is fellow of Indian academy of Paediatrics she has worked with UNICEF and WHO and is National expert for human milk banking in India and technical expert group committee member for global guidelines she has published 176 papers, she currently the Dean at MGM Medical College, Nerul Navi Mumbai
